OUR OFFICES ARE LOCATED IN THE DALLES AT 312 E. 4TH ST AND HOOD RIVER IS AT 205 WASCO LOOP #101 (behind Wal Mart)

What items should I bring to my appointment?

  • identification for all adults in the households.
  • names and birthdates for all household members.
  • social security cards for all household members.
  • income verification for the past 30 days for all household members.
  • your most current heating bill and your electric bill.
  • if you receive Social Security, your most recent benefit letter.
  • your water and garbage bill for the available discount programs???????
